Leave of Absence (LOA) 

At Doma, we understand that life events sometimes require time away from work. Depending on the situation, you may be eligible for a leave of absence under company policy, federal law, or state law.

Common Reasons for a Leave of Absence

You may qualify for a leave for reasons such as:

  • Your own serious health condition

  • Caring for a family member with a serious health condition

  • Birth, adoption, or foster placement of a child

  • Military service or military family needs

  • Bereavement (death of a family member)

  • Jury duty

  • Voting in elections

  • School conferences or activities

  • Organ or bone marrow donation

  • Domestic abuse, harassment, or crime victim-related court proceedings

  • Religious observances

Family & Medical Leave (FMLA)

Eligible employees may qualify for up to 12 weeks of unpaid, job-protected leave (or up to 26 weeks for certain military caregiver situations) under the Family and Medical Leave Act (FMLA).

To be eligible for FMLA, employees generally must:

  • Have worked for Doma for at least 12 months

  • Have worked at least 1,250 hours during the previous 12 months

  • Work at a location that meets FMLA coverage requirements

While on approved FMLA leave, eligible employees may continue health insurance coverage under the same terms as active employees.

Minnesota Paid Leave (Beginning January 1, 2026)

Minnesota's Paid Leave program provides eligible employees with paid time away from work for certain family and medical reasons. This state-administered benefit may provide partial wage replacement when employees need time off for qualifying events, including:

  • Caring for a serious health condition

  • Caring for a family member with a serious health condition

  • Bonding with a new child through birth, adoption, or foster placement

  • Certain military-related family needs

  • Personal safety leave related to domestic violence, sexual assault, or stalking

 

Eligibility, benefits, and duration of leave are determined by the Minnesota Paid Leave program and applicable state regulations.

Employees who believe they may qualify should notify their supervisor and Human Resources as soon as possible and complete the Doma Leave of Absence Request Form. Human Resources can help employees understand how Minnesota Paid Leave may coordinate with other leave benefits, including FMLA and company leave policies.

Requesting a Leave

If you know you will need time away from work, please notify your supervisor and Human Resources as soon as possible. Some types of leave require advance notice and supporting documentation.

How to Submit a Leave Request

To help us process leave requests quickly and consistently, Doma uses a Leave of Absence Request Form through Google Forms. 

Employees requesting a leave of absence should:

  1. Notify their supervisor of the anticipated leave.

  2. Complete the Leave of Absence Google Form.

  3. Submit any required supporting documentation (such as medical certification, if applicable).

  4. Respond to any follow-up requests from Human Resources.

Human Resources will review the request and communicate next steps, eligibility requirements, and any documentation needed.
