Encouraging Growth and Independence
Doma understands that the continuum of care will change throughout a person’s life. Therefore they may need unique services and housing options, depending on their circumstance and we look forward to exploring this together. We are committed to providing an array of community living options that encourage growth and pathways toward independence.
Adult Family Home (AFH) - Wisconsin
Adult Family Homes are waivered services provided to people 18 and over who have been deemed eligible by the Aging and Disabilities Resource Center. These homes can provide around the clock care and be tailored to meet each individual’s needs.
Board and Care Facility - Minnesota
Doma offers a Medicaid Certified Board and Care with 24 hour nursing support. This setting is licensed by the Minnesota Department of Health and is a good fit for individuals who need an intermediate level of care.
Support in your own home-Cleveland
At Doma OH, we believe everyone deserves the opportunity to live life on their own terms. Some people need a few hours per day while others need 24/7 staffing.
We proudly support individuals with disabilities in Cleveland, Ohio, through Medicaid waiver services, helping each person create a safe, comfortable home and a life filled with possibility.
Our team goes beyond daily care. We connect people with supportive landlords, assist with medications and medical appointments, and encourage participation in social and leisure activities that bring joy and connection.
Through our Homemaker/Personal Care (HPC) services, we walk alongside each person, providing the support they need to live independently while pursuing their goals, building friendships, and being part of the community they love.
